Immunization Records

Each student should present a completed Kansas Certificate of Immunizations (KCI), when first enrolling at Christ the King School. This is proof that each student has completed all tests and vaccinations that are deemed necessary by the Secretary of the Kansas Department of Health and Environment. Immunization laws exist to reduce the spread of communicable diseases and to protect those who are unable to get vaccinated.

Required vaccinations for kindergarten through 12th grade (K-12) and early childhood programs for 2024-2025:

  • DTaP (Diphtheria, Tetanus, Pertussis)
  • Hepatitis A
  • Hepatitis B
  • MMR (measles, mumps, rubella)
  • Meningococcal ACWY
  • Polio
  • Varicella (chickenpox)
  • Hib (Haemophilus influenzae type b) – early childhood programs only
  • Pneumococcal conjugate (PCV) – early childhood programs only

Failure to have these tests and inoculations or complete them in a timely manner shall be deemed non-compliance, and schools may reserve the right to deny admittance.

Immunization Exemptions

In Kansas, government and school authorities may legally grant exemptions to immunizations for two reasons: medical and religious. Below are the requirements to seek either exemption at Christ the King School.

Medical Exemption - a KCI - Form B signed by a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.)  KCI - Form B (Medical Exemption).pdf

Religious Exemption -  Although the Catholic Church supports and encourages vaccinations, it is not a moral obligation imposed on every member of the Church. It must be voluntary,
and not obligatory by mandate. The Diocese supports the rights of the parents to make informed medical decisions for their children. Therefore, the Catholic Diocese of Wichita grants the religious exemption to parents who choose not to vaccinate their children.  Annual religious exemption shall be documented on the following form, before the first day of school each year.
Religious Exemption Form (Diocese of Wichita) 2026

In the case of a vaccine-preventable disease outbreak in the school or exposure to a vaccine-preventable disease, students with an exemption (either medical or religious) will be excluded
from school until the danger of the outbreak has passed, as determined by local public health officials, or the student has been immunized. Exclusion Periods for Common Vaccine-Preventable Diseases ranges from twenty-one days to twenty-eight days.
